Workplace injury or illness incident reporting procedure

Department reporting procedure

  1. Dial 911 if emergency services are needed.
  2. The injured person’s supervisor—or a stand‑in if the supervisor is unavailable—must submit an online Incident Report as soon as possible (required within 24 hours). Report immediately if the incident involves a fatality, in‑patient hospitalization, amputation (including fingertip), or loss of an eye.
  3. After submitting the Incident Report, you will receive an email copy. Forward this email to the Department Chair and ansci.safety@wsu.edu.
  4. If the injured person is NOT an employee, skip ahead to Step 11. Otherwise, continue to Step 5.
  5. Immediately report the injury to EH&S at 509‑335‑3041 and notify the employee’s supervisor, if applicable.
  6. If the employee sustained a serious injury, continue to Step 7. Otherwise, skip ahead to Step 10.
  7. The supervisor or stand‑in completes the online Supervisor’s Accident Investigation Report, and optionally the Witness/Injured Person Statement.
  8. When you receive a copy of the completed report, forward it to the Department Chair and ansci.safety@wsu.edu.
  9. Provide requested information to unit and college safety committees, EH&S, and HRS.
  10. If medical treatment was necessary, if the employee was unable to work the next shift, or if the incident was a near miss that could have been serious, ensure that Steps 7, 8, and 9 have been completed.
  11. End of procedure
